Congenital duodenal obstruction(Neonatal surgery-8)


Abstract
Bile tinged vomiting of newborn with double bubble on simple erect film suggests congenital .duodenal obstruction. These include 1) atresia and stenosis, 2) diaphragm, 3) annular pancreas, and finally mid gut volvulus and Ladd¢¥s band. Volvulus of midgut and Ladd¢¥s band is due to anormaly of intestinal rotation and this will be discussed separatedy.
1. General incidence of 1 in 10,000 to 20,000 birth recorded.
2. Of 503 patients collected by Fonkalsrud during 10 years period from 65 major institution in America, there were 254 (48.7%) atresia, 206 (41.0%) diaphragm, and 50 (10.0%) stenosis. Besides, 106(21%) patients have annular pancreas. Annular pancreas seems to be a factor producing obstruction, in half (56 patient) of this patient.
3. Over 30% of the patient have Mongolism. Other anormaies are also frequently (24-37%) observed in these patient.
4. Duodeno-jejunostomy or duodeno-duodenostomy is treatment of choice in most of the patient.
5. Pathogenesis of the disease,. including Tandler¢¥s, Louw¢¥s, Sutton¢¥s and Lecco & other¢¥s are discussed.
6. Cases of duodenal stenosis, diaphragm and annular pancreas are illustrated.
